Treatments: temporary or permanent fillers. We live in an exciting time where the variety of fillers and volumizers allows the safe and effective correction of volume loss and wrinkles over the entire face. Bovine collagen, for decades the only available injectable agent, has now been largely replaced by other, mostly synthetic compounds that have a variable live span and indication. They can be used to correct static lines over the entire face, to lessen dark circles under the eyes, augment lost volume in the cheeks and in the lips, and plump up the back of your hands for rejuvenation.

With the exception of one (Artefill), they get degraded by the body’s own enzymes over periods of 3-24 months and do not require skin testing prior to the procedure. People who like the effects of the temporary fillers and want to reduce the number of injections over time may choose Artefill, the only permanent injectable filler and volumizer. Because its filler component is collagen-based, it needs a skin test one month prior to injection.

Unwanted redness usually occurs on the face, neck or chest of fair skinned individuals who have had chronic sun exposure. When paired with visible blood vessels and brown freckling it is referred to as poikiloderma.

IPL treats red and brown at the same time but many treatments are required and results are usually not lasting. Redness and blood vessels can be targeted very effectively and permanently with pulsed dye laser treatments. For brown freckles, several treatment modalities can be added on the same day. Sunscreens with a broad UVA range should be used meticulously and for life to sustain your results.

So, the decision of skin rejuvenation treatment must be extremely personal and tailored to achieve your goals. Skin rejuvenation offers several avenues to restore freshness and vitality of youth.

Skin Maintenance and Chemical Peels

Some of us are simply interested in a refreshed look. This can be easily achieved by daily use of sunscreens and topical retinoids at night (if tolerated). Occasionally, we need to promote turnover of skin cells by targeting the top layer of skin using the time-tested method of peeling. Gentle chemical peels made of glycolic acid or more potent TCA peels can improve appearance by removing some superficial skin to erase dullness. Another method of exfoliation, microdermabrasion uses ultra-fine aluminum oxide crystals to selectively remove dead layers of skin. This process simultaneously stimulates production of elastin and collagen to effectively promote development of a firmer, more youthful looking skin.

As we progress through life, we might want ‘to turn back time’ and look 10 years younger. Fortunately, treatment options for this dramatic rejuvenation are manifold and can be tailored to one’s goals, age, lifestyle and even ability to tolerate downtime. A treatment plan may incorporate several different modalities: skin resurfacing techniques; laser removal of pigmented spots; laser skin tightening; strategic use of synthetic fillers, volumizers and Botox injections.

Skin Resurfacing for Dramatic Results

Skin resurfacing devices, like the paragon carbon dioxide (CO2) laser, effectively deliver dramatic results. They use controlled damage to trigger the skin’s healing response, a process that eventually leads to skin remodeling and improvement in various signs of aging. Laser resurfacing offers a precise, effective and long-lasting approach to skin rejuvenation. Since this technique directs short and concentrated pulsating beams of light at irregular skin, laser skin resurfacing removes skin very precisely. Specially designed pulsed CO2 lasers can vaporize skin layer-by-layer while causing minimal damage to untargeted skin tissue. While fractional resurfacing with Fraxel uses a very narrow column of light to heat layers of the skin to stimulate collagen production, the cool light of erbium laser treatments facilitate removal of uneven pigmentation and superficial roughness from aging and sun-damaged skin. Plasma resurfacing is a relatively new technology that delivers plasma, a high energy ionized gas, through the epidermis into the deeper layers of the skin where collagen production is stimulated. Indeed, CO2 lasers, erbium lasers and plasma devices can deliver dramatic results in a fractional, ablative or non-ablative manner with downtime inversely correlated to the number of treatments needed.

Skin Tightening

Skin tightening will further enhance the effects of skin resurfacing and are built in with CO2 laser treatments. When length of healing is a concern, radiofrequency can be used to tighten the skin, albeit the results may not be as reliable or long lasting.

Skin Luminosity – Brown, Sun, and Age Spots Treatment

Luminosity of the skin has been shown to directly correlate with a youthful appearance. Most rejuvenation plans therefore incorporate removal of brown/age/sun spots. If downtime with CO2 or erbium laser resurfacing are unacceptable, treatments including a range of laser or non-laser intense pulsed light sources (IPL) may be chosen. Many skin problems of adulthood, such as fine lines, blotchiness, age spots (liver spots, dark spots), surface roughness, wrinkles, and dull, lifeless skin, can be attributed to sun exposure. The unique ability of specific lasers and IPL to specifically target pigment in the skin results in safe, effective and rapid treatment of unsightly skin lesions that have brown pigmentation.
Wrinkle Treatment – Fillers – Botox , Restylane, Elevesse, Radiesse, and others

Stubborn wrinkles and fine lines often require more help than is available at the cosmetics counter. Modern technology has provided an array of fillers and volumizers that can be used over the entire face to strategically and effectively correct static lines and volume loss. In recent years, several new skin fillers have received approval by the U.S. Food and Drug Administration (FDA) for treating facial wrinkles and are safe and effective options for combating aging skin. Wrinkles are thought to result from the loss of three crucial skin components: collagen, elastin and hyaluronic acid through external and internal ageing. Cosmetic injectables offer a variety of safe, non-invasive ways to subtly optimize one’s appearance. Injection procedures with synthetic fillers have revolutionized aesthetic medicine and made cosmetic enhancements safe, easy and affordable. Of particular interest, the synthetic fillers (e.g. Restylane/Juvederm, Elevesse, Radiesse, Artefill and Sculptra) represent a new generation of alternatives to animal-based collagens. These newer products are more versatile in their weights and densities, biocompatible and longer lasting. Injectable fillers can be used to round out wrinkles, lines, and scars, or even to subtly give the face a more desirable shape.

In certain cases, treatment plans might also take advantage of removers of dynamic lines, like Botox, and fillers for volume restoration. Botox cosmetic acts as a safe, reliable medication, which blocks the actions of certain nerves. For wrinkles this means relaxing the muscles that crease the skin or pull the skin down.

What is laser skin tightening

Laser skin tightening is a minimally or non-invasive process that uses an infrared light source (a laser) to tighten skin by heating the collagen under the skin’s surface, causing the skin to contract (tighten).

Skin tightening is a unique option for facial and body rejuvenation that produces dramatic results. Skin tightening effectively reduces the appearance of loose skin or fine and intermediate wrinkles for a smoother and more youthful appearance. Sometimes, surgical correction (face lift, tummy tuck etc) is the only option. However, most of the time, laxity of the skin is mild or moderate, and a less invasive treatment can achieve the desire results.

Who can benefit from skin tightening

People who start noticing loose skin in their cheeks, neck, chest, abdomen, buttocks, legs, and arms as a result of aging or weight loss could consider skin tightening treatment. Laser skin tightening is appropriate for men and women of all skin types and skin tones who wish to achieve dramatic results without painful surgery and lengthy recovery.

Skin tightening expected results

Skin type, body area, and downtime play a role in the outcome. Also, various laser technologies provide different degrees of skin tightening. In general, the more aggressive the treatment the more noticeable the results are.

CO2 laser treatment is the most aggressive approach and is usually used when skin tightening and skin rejuvenation are desired. It delivers the most drastic results. It is effective for the treatment of the face, and neck in fair and Asian skin.

A newer technology that is based on the CO2 laser is fractionated CO2 laser (DeepFX). It delivers similar results for skin tightening with greatly reduced downtime. In addition, it treats effectively other body areas such as the chest and the lax abdomen after pregnancy.

Titan laser can treat all body parts. In comparison to CO2 laser treatments, the downtime is much shorter but it requires multiple treatments to achieve substantial results. Results can last up to 2 years and additional treatment can follow to maintain the desired look.

Nd:YAG, Thermage™ ( radiofrequency), ReFirme™ are also used for tightening lose skin. These products use lower energy levels to deliver reduced patient discomfort. Consequently, multiple treatments are required to achieve significant results.

Skin tightening down time

If the 7-14 day-downtime of the conventional CO2 laser is unacceptable, fractionated CO2 with a downtime of 3-6 days may be used with minimal sacrifice of results.

Nd:YAG, Titan, Thermage™ do not harm the surface of the skin. Therefore, those treatments do not involve downtime. However, allocating time for multiple treatments should be considered.

Skin tightening pricing

Skin tightening cost depends on the desired results (number of treatments) and the size of the treated area and can vary between $500 to $7000.

CO2 laser or fractionated CO2 laser resurfacing require usually a single treatment with an average cost of $3000.

Thermage™ and Titan treatment cost structure is similar. A single session usually costs around $1500. As two to three treatments are usually needed, a patient should expect an average cost of around $4000.

True acne scars can only be improved by removing the top layers of the skin and letting the skin build a new surface combined with maximum collagen production from underneath. This can be achieved by CO2 laser resurfacing. The laser assures precise depth of abrasion, thereby minimizing complications that were common with dermabrasion (not microdermabrasion), such as permanent pigment change.

Deep pitted scars require punch grafts 4-6 weeks prior the laser procedure for maximum fill. Nowadays, fillers can be used to bolster scars. Often, acne scars may be preventable if acne treatment is started early.

Most individuals with rosacea experience blushing or flushing over the central face with trigger factors such as sun exposure, drinking hot beverages or alcohol or eating spicy food. Persistent central facial redness is often present. The underlying enlarged vessels can very efficiently be treated by vascular lasers, greatly improving background redness as well as diminishing flushing with rosacea triggers.

Inflammatory rosacea presents with red papules or pustules and requires topical or oral anti-inflammatory agents, or when recalcitrant, photodynamic therapy. Men may develop an overgrowth of tissue on the nose in response to long-standing rosacea that can be corrected by CO2 laser ablation.

Ocular rosacea may be misdiagnosed if rosacea does not affect the skin. Oral anti-inflammatory antibiotics may be crucial to prevent possible destruction of eyesight.

These benign, visible blood vessels are common and may occur on the face or trunk and may be unsightly to some people. Laser is the only non-scarring treatment option.

One treatment is typically enough when angiomas are flat. If they are raised they require multiple laser or electrocautery treatments. Larger angiomas may have to be excised.

Sclerotherapy is a technique designed to close unsightly spider veins on legs, face or other body areas. Several substances can be injected to cause this outcome. While providing identical results and downtime, detergents/foaming agents are generally considered painless while hypertonic saline injections are somewhat painful. Sclerotherapy is used for the treatment of larger spider veins. It is often followed by sclerolaser treatments which resolve extremely fine vessels that did not take to sclerotherapy.

Depending on the examination, ultrasound may be necessary to exclude reflux, which may hamper treatment success. If reflux is detected, a consultation with a vascular surgeon may be necessary to remove underlying varicosities.

It is safe to say that all of us, some more than others, suffer from unwanted hair on our face, neck and body. This unwanted hair growth can occur due to heredity, hormonal levels, and certain drugs.
Who can benefit from Permanent Hair Removal?
Usually, hair growth is desirable; however, when the hair is on the wrong part of your body i.e. a woman’s upper lip or chin as well on your hands or arms then you may be considering the help of permanent hair reduction.

Laser hair reduction works best on dark hair. It should be noted that at this time neither lasers nor IPL (intense pulsed light) are able to treat red or white hair.
There are several methods of hair reduction available, both temporary and permanent.
Temporary Hair Removal

Temporary methods of hair removal are achieved through depilation (removal of the hair above the skins surface) and epilation (removing the entire hair). Depilation refers to methods such as shaving and creams that chemically dissolve the hair. Epilation refers to methods such as tweezing and waxing. These methods remove unwanted hair for a period of a few weeks.
Permanent Hair Removal – Laser
Laser hair removal is the most effective method for permanent hair reduction. Laser hair reduction works well on dark hair in people of all skin types. On average, 20-25% of hair follicles are permanently destroyed after each laser treatment, leading to about 50-70% reduction after three treatments and about 80-90% reduction after six treatments. The remainder of the hair then becomes finer and lighter. However the most common misconception about permanent hair reduction is that a series of laser treatments destroys all hair when in reality some hair follicles are dormant at the time of treatment and are able to produce hair at a later stage. For near complete hair removal, periodic maintenance treatments can be performed.
Permanent Hair Removal – Electrolysis

An alternative method of permanent hair removal is electrolysis. This technique employs an electrical beam to destroy a single hair follicle. Therefore, it is unsuitable for large body areas. Electrolysis can eliminate hair of all color, including white, and red. Electrolysis is often used to remove the occasional gray or white hair that is left after successful laser treatment of laser hair reduction.
Pre and Post Permanent Hair Removal Instructions
If you are planning to undergo permanent laser hair reduction, the hair must not be plucked or waxed for four weeks prior to treatment. Shave your hair (with a new blade) before coming to the office. Once the treatment has been administered, you should avoid long periods in direct sunlight for about six weeks.
