outcomes.\u00a0 For this reason, we offer a scheduling guarantee for all patients referred to us from a doctor&#8217;s office using referral form provided.\u00a0 This guarantee ensures that all patients who are referred from a physician&#8217;s office will be seen within 72 hours of the referral.<\/p>\n<p>&nbsp;<\/p>\n<p>For your convenience, we have created the following referral
